Schedule and availability is currently all done through Booksy. Create an account and download the Booksy app to schedule your appointment.

Availability & Cost

Appointments are currently being scheduled on Wednesday’s from 1:15pm-6:00pm, and select Saturday’s from 9:00am-2:00pm. Availability can be viewed through the Booksy app. Cost is $125 for a 50-minute session, and payable at the conclusion of your appointment. Credit card, HSA/FSA, and cash are accepted forms of payments.

There are THREE forms you’ll need to complete before attending your session:

  • Intake Form (Adult or Pediatric)

  • Informed Consent Form

  • HIPPA Form

After scheduling your appointment, I will email you ahead of time to discuss your needs and expectations. I will send you a copy of the forms to complete prior to attending your scheduled session. These forms are also available to download below by clicking the buttons. Please compete your forms and PRINT the finished copies to submit when you attend your appointment. Thank you.
